Confusional syndrome as a form of presentation of infected bulla in an elderly patient

Abstract

Introduction: Infected bullae are a complication that can occur in patients with emphysematous chronic obstructive pulmonary disease. They usually present with fever, shortness of breath, cough, and pain in the side; therefore, disorientation is rarely present as the main symptom.
Objective: To describe the clinical and imaging characteristics of a patient with confusional state secondary to an infected bulla.
Clinical case: An 82-year-old male patient with a history of heavy smoking and emphysematous chronic obstructive pulmonary disease. Was brought to the emergency room due to disorientation, weakness, and loss of appetite. An infected bulla associated with confusional state was diagnosed by computed tomography. Antibiotic therapy was prescribed, and the patient's condition progressed favorably with resolution of clinical manifestations.  
Conclusions: Confusional syndrome secondary to an infected bulla should be considered in the differential diagnosis of elderly patients with a history of smoking and pulmonary emphysema. The early diagnosis and treatment greatly improve the prognosis.
