Teleradiology for the development of diagnostic imaging skills in Family Medicine residents

Abstract

Introduction: The limited interpretive capacity in imaging diagnosis affects the quality of care in Cuban primary health care, demanding virtual training strategies for medical residents.

Objective: To implement a virtual course with Family Medicine residents for the development of diagnostic imaging skills.

Methods: The research had a pre-experimental design with pre-intervention and post-intervention measurement in three phases: diagnosis, intervention, and evaluation. The sample consisted of 48 Family Medicine residents selected through intentional sampling. A comprehensive assessment of skills and needs was conducted, followed by a 12-week intervention with a virtual course adapted for low connectivity. The impact was measured through standardized tests, statistical analyses, and qualitative approaches that evaluated diagnostic accuracy, unnecessary referrals, and participant perception.

Results: Significant improvements were observed among residents, with a 28% increase in accuracy and a reduction in unnecessary referrals. A strong correlation (r=0.76; p<0.001) was found between simulation hours and clinical skills, with an ideal threshold of 3.5 hours/week. Although the program was valued, technical challenges were faced in rural areas. Third-year residents achieved greater clinical transfer, although studying through small devices was limited.
Conclusions: The virtual course improved diagnostic imaging skills, reduced unnecessary hospitalizations, but its success requires investments in infrastructure and adaptations according to the level of training.
