Ectima múltiple bilateral en miembros inferiores: presentación infrecuente en una paciente diabética / Bilateral multiple ecthyma of the lower limbs: uncommon presentation in a diabetic patient

Introducción: El ectima es una piodermitis extrafolicular profunda infrecuente, reconocida como la forma ulcerativa del impétigo contagioso.
Objetivo: Describir la evolución clínica y abordaje terapéutico del ectima múltiple bilateral en miembros inferiores en una paciente diabética.
Caso clínico: Paciente femenina de 45 años, con diabetes mellitus tipo 2, acudió por lesiones dolorosas en región posterior de ambas piernas de una semana de evolución. Al examen físico se observaron múltiples lesiones ulcerocostrosas, redondeadas y ovaladas, cubiertas por costras gruesas, negruzcas y adherentes. El exudado bacteriológico confirmó coinfección por Staphylococcus aureus y Streptococcus del grupo A, ambos sensibles a cefalexina. Recibió tratamiento médico y a los 15 días las lesiones cicatrizaron por completo, persistió hiperpigmentación postinflamatoria y cicatrices atróficas residuales.
Conclusiones: El ectima múltiple bilateral en miembros inferiores constituye una presentación infrecuente, cuyo reconocimiento se fundamenta en su morfología característica. La diabetes mellitus, como factor predisponente, condiciona tanto la extensión de las lesiones como la duración del proceso de cicatrización. Por tanto, el diagnóstico precoz y el tratamiento sistémico adecuado resultan esenciales para prevenir secuelas y complicaciones.
