Crisis glaucomatociclítica. A propósito de un caso.

Resumen:

La crisis glaucomatociclítica o síndrome de Posner Schlossman, es una enfermedad rara, que afecta a pacientes entre  20 y 50 años de edad. Caracterizada por una uveítis anterior  recurrente leve, unilateral, asociada a un aumento brusco de la presión intraocular (PIO), sin que aún su causa haya sido dilucidada, aunque existen varias teorías propuestas, que van desde autoinmune a infecciosa. El tratamiento se centra en controlar la PIO y disminuir la inflamación. Un ataque generalmente se resuelve sin secuelas, mientras que repetidos en el tiempo pueden conducir a daño glaucomatoso. Realizamos la presentación de un caso del sexo masculino de 45 años de edad que acudió al cuerpo de guardia refiriendo visión borrosa del ojo izquierdo. El examen reveló ojo blanco, reacción inflamatoria de la cámara anterior leve y una PIO elevada de dicho ojo. Se impuso tratamiento con antiinflamatorios e hipotensores tópicos y sistémicos, mejorando el cuadro en horas. Se notó que al suspender el antiinflamatorio esteroideo (de forma escalonada y muy lentamente), comienza a recaer presentando una nueva crisis. Se decide realizar cirugía filtrante (trabeculectomía) con antimetabolitos encontrándose un citomegalovirus en la muestra del humor acuoso por PCR. Esta afección constituye uno de los síndromes específicos de uveítis anterior hipertensiva el cual representa con frecuencia un reto diagnóstico y terapéutico.

The crisis Glaucomatociclítica or Posner Schlossman's syndrome,It is a rare disease, that he affects patients among 20 and 50 elderly years. The treatment focuses in controlling oneself her pressure intraocular and decreasing the inflammation. An attack generally gets worked out without sequelae, while than repeated in the time they can conduct to damage glaucomatoso. We accomplished the presentation of a case of the masculine sex of 45 elderly years that he was present at the guardhouse referring blurred vision of the left-handed eye. The exam revealed white eye, light inflammatory reaction of the previous light anterior chamber elevated intraocular pressure lifted of the aforementioned eye. Treatment with anti-inflammatory and hypo-tightening topics and systemics was imposed, improving the crisis in hours. Note him than when hanging the anti-inflammatory esteroideo ( of stepped form and very slowly ), he begins to relapse presenting a new crisis. Decides to accomplish filtering surgery with antimetabolits finding a citomegalovirus in the sign of aqueous humor for PCR. This affection constitutes one of previous uveítis's specific syndromes hipertensiva which represents a diagnostic and therapeutic challenge frequently.
